Pharmacist needed for clinical program for practice sites located in oklahomaOklahoma pharmacy license requiredThis program is part of pharmd communityWww.pharmdc.comThe ideal candidate should have experience in clinical encounters including medication management therapy, annual wellness visits, transitional care management, remote patient monitoring and the core clinical knowledge to support all chronic care management diseases statesTraining will be availableExperience and advanced credentialing preferred.
