Job description:
Job title: case manager - remote care status: non exempt

Supervised by: team leader
Job qualifications:
Bachelor’s degree in counseling, psychology, social work, or related mental health discipline
Prefer some experience working with individuals with mental illness
Ability to work independently and part of a team is required
Belief in the concept of mental health recovery
Essential duties and responsibilities:
Monitor assigned cases and update care plans by due date
Review cases for closure with supervisor
Review cases for referrals to higher level of care
Provide engagement and coordination calls to clients in case management and health home services
Complete documentation accurately and timely
Attend required meetings
Achieve targets related to care plan completion and participant contact
Compliance with annual kpi evaluation
Regular and reliable attendance is required
Other duties as assigned
